Seizures and epilepsy after intracerebral hemorrhage: an update

نویسندگان

چکیده

Seizures are common after intracerebral hemorrhage, occurring in 6–15% of the patients, mostly first 72 h. Their incidence reaches 30% when subclinical or non-convulsive seizures diagnosed by continuous electroencephalogram. Several risk factors for have been described including cortical location presence intraventricular total hemorrhage volume, and history alcohol abuse. may theoretically be harmful as they can lead to sudden blood pressure fluctuations, increased intracranial pressure, neuronal injury due metabolic demand. Some recent studies suggest that acute symptomatic (occurring within 7 days stroke) associated with worse functional outcome death despite accounting other known prognostic such age baseline volume. However, impact on prognosis is still debated it remains unclear if treating preventing might improved clinical outcome. Thus, currently available scientific evidence does not support routine use antiseizure medication primary prevention among patients hemorrhage. Only prospective adequately powered randomized-controlled trials will able answer whether seizure prophylaxis longer term settings beneficial

Seizures after Spontaneous Intracerebral Hemorrhage

OBJECTIVE In patients with spontaneous intracerebral hemorrhage (ICH), the risk factors for seizure and the effect of prophylactic anticonvulsants are not well known. This study aimed to determine the risk factor for seizures and the role for prophylactic anticonvulsants after spontaneous ICH. METHODS Between 2005 and 2010, 263 consecutive patients with spontaneous ICH were retrospectively as...

Pediatric intracerebral hemorrhage: acute symptomatic seizures and epilepsy.

IMPORTANCE Seizures are believed to be common presenting symptoms in neonates and children with spontaneous intracerebral hemorrhage (ICH). However, few data are available on the epidemiology of acute symptomatic seizures or the risk for later epilepsy. OBJECTIVE To define the incidence of and explore risk factors for seizures and epilepsy in children with spontaneous ICH. Our a priori hypoth...

The CAVE score for predicting late seizures after intracerebral hemorrhage.

BACKGROUND AND PURPOSE Seizures are a common complication of intracerebral hemorrhage (ICH). We developed a novel tool to quantify this risk in individual patients. METHODS Retrospective analysis of the observational Helsinki ICH Study (n=993; median follow-up, 2.7 years) and the Lille Prognosis of InTra-Cerebral Hemorrhage (n=325; 2.2 years) cohorts of consecutive ICH patients admitted betwe...

Neuroinflammation after intracerebral hemorrhage

Spontaneous intracerebral hemorrhage (ICH) is a particularly severe type of stroke for which no specific treatment has been established yet. Although preclinical models of ICH have substantial methodological limitations, important insight into the pathophysiology has been gained. Mounting evidence suggests an important contribution of inflammatory mechanisms to brain damage and potential repair...

Matrix metalloproteinases in human spontaneous intracerebral hemorrhage: an update.

BACKGROUND In default of a plausible and satisfactory causal treatment for hemorrhagic stroke, a role of matrix metalloproteinases (MMPs) in the pathogenesis of cerebrovascular diseases has recently been widely discussed. The well-known impact of MMPs on extracellular matrix destruction triggered by inflammation as a foundation for several diseases, including stroke, is very much in evidence. N...
